The Hospice, Palliative Care, and Homecare Social Worker functions as a member of the interdisciplinary team for Hospice, Palliative Care, and Homecare. The Social Worker provides clinical social work services to patients and their families enrolled in Hospice, Home-Based Palliative Care program and to select patients and their families receiving services from Homecare. The Social Worker provides initial psychosocial evaluations, psychosocial counseling and intervention, direct casework services, grief/bereavement services, and community education, outreach and referrals. The social worker also acts as the Hospice Volunteer Coordinator and/or Hospice Bereavement Coordinator. The Social Worker also provides consultation, education and training to home care staff regarding psychosocial issues that impact the delivery of optimal home care services.

Monday-Friday with one day off during the week
8am-5pm

Education: Masters degree in social work from an accredited school of social work required.
Licensure: Licensed with the State of Ohio Counselor, Social Worker, Marriage and Family Therapist Board. Valid Ohio driver license in good standing, proof of sufficient auto insurance, reliable personal vehicle required.
Certifications: Hospice and Palliative Care certification preferred.
Skills: Possess well-developed coping skills. Strong ability to problem-solve with families, staff and other community agencies.Experience: Two years of previous hospice, home health or medical social work experience preferred. Experience and skill in working with children, adolescents and families affected by chronic and/or life-threatening illness specifically in counseling and connection to resources.The above list of duties is intended to describe the general nature and level of work performed by individuals assigned to this classification.
