JOB SUMMARY:
The Community Health Advocate (CHA) is part of the Hospital's Behavioral Health Emergency team that provides a screening and assists with disposition plans for adult and pediatric patients that present in the ED with a mental health crisis. This position helps behavioral health patients connect with resources that allow them to better manage their mental illness, improve their overall health and avoid preventable hospital encounters. The CHA is an integral part of the multi-disciplinary team that includes the ED staff and psychiatric Social Workers. The CHA embedded in the ED is responsible for assisting with post discharge care and connecting patients to community resources and supports for individuals with complex needs. This position also provides post-discharge follow up with patients seen in the ED for mental health crisis in order to promote and support participation in referrals for outpatient treatment and other community-based service referrals made while in the ED.
