MUST:
Licensed Social Worker in the state of Maryland
Comfortable working with long term care residents
Experience dealing with crisis intervention support
Active CSW/MSW Maryland license
Bachelors degree in social work
DUTIES:
Coordinate stay through the admissions and discharge process
Provide crisis intervention support
Provide assistance related to disease, hospitalization, financial and educational needs
Work with residents so they understand their rights
Document information in patient files
