Posting Summary

We're growing and adding another Perinatal Care Coordinator to the DC Mother-Baby Wellness team. An integral member of the DC Mother-Baby Wellness Program, the Perinatal Care Coordinator (PCC) is an advocate, a resource and the first contact patients will have with the DC Mother-Baby Wellness Program. The Perinatal Care Coordinator welcomes patients to the program and provides care coordination support for families experiencing difficulties getting needs met; ensures optimal patient care and supports excellent collaboration between teams. The ideal PCC candidate has experience working with pregnant and post-partum people, strong organizational, administrative and computer skills, and excellent verbal PCC and written communication skills. Bilingual and/or prior experience working in a perinatal health setting preferred. Prior experience in health care required.

The Perinatal Care Coordinator functions as a resource and advocate for pregnant and post-partum women. Conducts maternal mental health screenings and systematically reviews screening data, provides appropriate responses for identified needs around social determinants of health, collaborates with all involved teams and partners to ensure that patient and family needs are met. Provides care coordination support for families experiencing difficulties getting needs met; ensures optimal patient coordination, excellent collaboration between teams, with a focus on family experience and outcomes. Bilingual Spanish is strongly preferred for this position.

Qualifications

Minimum Education
Associate's degree in social work, nursing, education, health sciences or related field is required (Required)
Bachelor's degree in Social Work, Nursing, Education, Health Sciences or a related field is preferred (Preferred)

Minimum Work Experience
2 years' experience in a behavioral health or other health care setting as a member of a multidisciplinary team providing care coordination services to patients. (Required)
1 year Experience working with pregnancy and/or post-partum patients, preferably providing care coordination services. (Preferred)

Required Skills/Knowledge
Excellent communication skills required.
Ability to work with diverse patient populations.
Ability to provide culturally appropriate trauma informed care and communication.
Knowledge of resources available to meet the diverse needs of families in Washington DC

Functional Accountabilities
Professional Practice/Research

  • Identifies patients/families appropriate for Care Coordination services in a systematic manner.
  • Completes ongoing staff education regarding coordination of care, culturally sensitive care, compassion fatigue, and other relevant topics.
  • Participate as part of a multi-disciplinary team providing maternal mental health screening and response.

Structural Empowerment

  • Follows through on any service needs or requests until complete and satisfactory resolution; updates the team as needed.
  • Prioritized patient needs in the areas.
  • Support linkages between patients, the health care system and social services resources.

  • Completes documentation of all relevant conversations, meetings, and referrals.
  • Note and, if appropriate, escalate patent concerns or needs to the appropriate person or service.

    • Participates in the research related to identifying the best maternal mental health screening and response tools and protocols for optimal patient experience and outcomes.
    • Provides regular updates to the program team regarding family support and patient status.

    • Acts as a liaison among the DC Mother Baby Wellness Initiative team, medical providers and social service partners.
