I. JOB DESCRIPTION SUMMARY:
The Hospice Admissions Nurse reports to the Admissions Team Manager. The Hospice Admissions Nurse assesses patients in their place of residence or in hospitals and prepares the patient for entry into the spectrum of services provided by Gilchrist Hospice Care. The Hospice Admissions Nurse serves also as ambassador for Gilchrist Hospice Care within the service area. The Hospice Admissions Weekender works two 12-hour weekend shifts and is paid an eight-hour bonus; this position is also eligible for full-time benefits.
Education |
Diploma, ADN and/or BSN from an accredited institution |
Experience |
Three years of nursing. Home Health or Hospice experience preferred but not required. |
Skills |
* Knowledgeable of universal precautions for infection control and practices safe nursing standards for self protection * Strong time management skills and the ability to function with a changing daily schedule. * Excellent interpersonal and communication skills * Ability to be patient and family focused and provide patient/caregiver education * Skill in the use of EMR (Electronic Medical Record) |
Licensures, Certifications |
* Current MD nursing license. Expectation to obtain CHPN within two years. |
Physical Requirements |
* Ability to stand and walk almost constantly up to ninety 90% of work time and lifting and positioning patients up to fifty 50% of work time. * Ability to perform visits anywhere within the Gilchrist Hospice Service area. * Ability to concentrate and pay close attention to detail and often in stressful situations. |
Working Conditions |
* Normal office environment with driving as large part of the day in the field, which includes visits to hospitals, skilled nursing & assisted living facilities, and patient's homes, some in unsafe areas (escort provided). * Exposed to adverse working conditions due to performance of patient care activities and exposure to communicable diseases |
