The Comprehension Center for Bronchopulmonary Dysplasia (C4A) at Nationwide Children?s Hospital is unique in the world. It is the only impatient NICU setting where the focus of care is moderate to severe BPD. Patients are referred to the unit from the acute care NICU at NCH as well as the surrounding NICU?s in the Columbus area, the state, the region and occasionally from more distant parts of the country. Patients are transferred to the unit at near term, term or post-term age. Their medical history and progress at this point is increasingly complex and involves complicated care plans and close collaboration between nursing, ANP, Dietary, OT/PT, Music Therapy, RT, Care Management, Psychology, and social work as well as consultation/collaboration with Cardiology, Pulmonary, Endocrine, and ENT. Children with moderate to severe BPD continue to require care both inpatient and later in our outpatient BPD clinics for several years.
Preventable adverse healthcare outcomes are associated with fragmented patient care. This can be reduced by providing effective consensual respiratory care that is uniquely individualized for each and every patient. In order for our unit to continue to succeed in growth and development, it is essential for the Respiratory Therapy Department to develop a core team of consistent specialized RTs that are able to develop guidelines and policies regarding our unique modalities of individual consistent respiratory care. By developing a fundamental dedicated team of BPD specific Respiratory Therapists in the CCBPD we aim to provide a more effective, collaborative, and accurate respiratory care to our patients to produce the best outcome possible.

1. Reviews the patient chart for current respiratory care orders. Reviews other patient data (e.g., PFTs, ABGs, and history) to ensure appropriate planning and administration of ordered therapy.
2. Assesses the patients cardio-respiratory status using techniques such as inspection, auscultation, lab values and X-rays to aide in the delivery of ordered therapy.
3. Assesses and documents the effectiveness of ordered respiratory care treatments. Monitors responses to therapy using clinical parameters such as HR, RR, color, WOB, and LOC.
4. Works closely with physicians and other health care team members who specialize in the BPD respiratory management and establishes a framework of guidelines of alternative modes of therapy when applicable.
5. Facilitates and coordinates quality respiratory care in a cost-effective, timely manner while promoting multidisciplinary collaboration between the health care team members, including education of respiratory care BPD, guidelines for care, and new initiatives on the unit by attending Subgroup, Big Group, etc.
6. Administers IPPB, PD&P, aerosol therapy, pulse oximetry, peak flows, oxygen, humidification therapy and other airway clearance devices. Delivers medications primarily by the aerosol route. Accurately documents all therapy performed.
7. Sets up, calibrates, and troubleshoots ventilators and O2 delivery devices. Assists in the cleaning and processing of equipment.
8. Performs ventilator management in all areas. Works with volume, pressure ventilation. Monitors ventilatory volumes, pressures, inspired gas temperatures, oxygen concentrations, the patient?s airway and the ventilator/patient interface. Advises the physician of changes in patient status and recommends changes. Accurately documents ventilator parameters or other performed therapy.
9. Performs ventilator transports within the hospital. If requested, may perform a transport with the transport team.
10. Responsible for the order entry and billing of services on the hospital computer system.
11. Assists in arrangements for home care patients to include patient/parent teaching, review of equipment, and cleaning techniques.
12. Accepts verbal orders from physicians that pertain to Respiratory Care and/or Patient Care Services policies and procedures in accordance with the verbal order policy of the Patient Care Services Department.
13. Assists with nasotracheal and endotracheal intubation. Is present to assist with all tracheostomy tube changes, replacement or removal.
14. Provides age-appropriate basic and intermediate care to patients of all ages (neonates to adults).
15. Fulfills annual continuing education requirements for state licensure.
16. Performs other related duties as assigned
17. Works closely as a core team of therapists, with consistent and accurate hand offs between shifts to decrease variability of care.
Knowledge, skills and abilities required:
1. Registered Respiratory Therapist credentialed through the NBRC with licensure to practice in the State of Ohio by the Ohio Respiratory Care Board.
2. Associate's degree from and AMA-accredited Respiratory Care program.
3. Must possess and maintain current CPR card, and obtain and maintain NRP and PALS certification.
4. Effective communication and interpersonal skills required in interactions with patients and parents, and hospital staff, and a belief in interdisciplinary collaboration.
5. Ability to work under general supervision.
