Project Manager- PFK Population Health Partnerships

The Project Manager is responsible for support and coordination of the work carried out by Partners For Kids' (PFK) community provider-facing teams including Patient Outreach, Pharmacy Support, Provider Relations, and Quality Improvement.

Responsibilities

* Completes strategic analyses and develops business plans for PFK initiatives aimed at supporting providers to improve health outcomes of the children PFK serves.
* Develops and calculates metrics to track success of projects, department goals, strategic plan milestones, and balanced scorecard goals for the provider-facing teams. Develops dashboards and other tools to present and disseminate this information.
* Provides project support for teams responsible for the Foundational Elements of PFK's strategic plan.
* Manages meeting/working group infrastructure for all PFK community provider-facing teams including developing agendas, documenting meeting conclusions, timelines, resource needs, and data needs.
* Coordinates educational events, appreciation efforts, continuing medical education requirements, and other special projects as assigned.

Qualifications

Education: Master's degree in Hospital/Health Administration, Business Administration, Planning, Public Health, or other related field highly preferred.

Skills:
* Strong project management skills. Excellent communication and organization skills to effectively interact with providers, consultants, external professionals, administrators, and employees at all levels.
* Proficiency with spreadsheets, databases, and discussion facilitation tools (e.g. Stormboard and Mural). Strong skills in Microsoft Office Suite.
* Understanding of HEDIS measures and Medicaid billing data including DRG and CPT coding.
* Analytical skills to assemble, evaluate, and understand large and complex information and studies. Ability to identify current and future trends.
* Experience with Agile, preferred.

Experience:
* Three years of academic and/or professional experience in systems planning required. Health care experience, preferred.
* OR three years of experience in health care business development and physician relations, required.
