Community Connector I
Montgomery County based Supported Living program for adults with developmental disabilities is searching for a high energy, hands on, self starter to lead a community based team. Primary responsibility is to proactively implementing systems to ensure quality services for consumers in his/her home to include health, safety, and community participation. He/she will assist in coordinating the activities of the home by eliciting the preferences and needs of and providing support to the consumers. Coordinate or provide assistance to individuals in the area of recreation activities and community resources in a variety of community opportunities. Research community resources, recreational activities, and provide assistance to consumers and staff to attend these events. Keep families, consumers, staff and supervisor abreast of all community opportunities. Responsibilities also include facilitating teams to support people to be productive members of their community.
Successful candidates will be creative, person centered, detailed oriented individuals, capable of multitasking.
Minimum Requirements BA/BS in related w/ 3 yrs exp. in related field.
Must have case mgmt, organization and leadership skills
