Position Description

Community Connector II

PT between hours of 7am and 3pm

Responsible providing quality supports to consumer. 

Must be energetic; must be familiar with Montgomery County area; must be a team player; must be creative; must be an independent thinker; must have a good driving record; must be willing to work flexible hours (day, night, and weekends); must be willing to travel in and outside the state when duty calls.

Minimum Requirements include high school diploma, valid driver’s license and ability to work independently.
