Position Description
Community Specialist (pt w/benefit 30hr position)
Basic Responsibilities
Provide necessary individual care and supervision to minimize risk while experiencing and exploring new community-based work and activities.
Instruct individuals to become more independent at work-related tasks, community-based activities and experiences (such as riding bus, shopping, etc).
Implement consumers’ individual plans and behavior plans.
Make recommendations to pull team together to discuss changes to the plan, or development of new goals, when needed.
Monitor consumer progress against agreed-upon goals, and document progress, as required, on documentation sheets.
Successful candidate will be highly skilled; able to work in an individuals home; able to administer medication; flexible to work Monday to Sunday put not to exceed 30 hours, be familiar with and able to travel about in Montgomery County.
Minimum Requirements:
Medication Certification, high school diploma, must have valid driver’s license and reliable transportation and ability to work independently.
Previous knowledge of quick profiles, behavior plans, Individual Plan, etc preferred.
