Job Description

Job Overview:

A TriHealth Clinical Pharmacist is responsible for providing optimal pharmaceutical care to all patients we serve through distributive and clinical functions. The Clinical Pharmacist collaborates with other health care providers to ensure appropriateness of all elements of pharmaceutical care and independently operates within policies and procedures to manage the department's clinical and consultative services. The Clinical Pharmacist supervises the work of supportive team members to ensure accuracy and efficiency of distributive services when working in a centralized role. Decentralized pharmacist roles have additional responsibilities include but are not limited to serving as a drug information resource for TriHealth providers, direct patient interaction, precepting of pharmacy residents and/or student pharmacists, and contribution to department projects or initiatives in collaboration with Pharmacy Leadership.

Job Requirements:

Bachelor's Degree in Pharmacy
Pharmacist
Working knowledge of all Ohio Board of Pharmacy regulations
1-2 years experience Pharmacy

Job Responsibilities:
Create and maintain an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm , professional environment.

  • Establishes effective collaborative working relationships and open and active communication with pharmacy leadership, physicians, nurses, pharmacists, and technicians.
  • Clearly articulates ideas and thoughts through written and verbal communications.
  • Promotes the correct and appropriate use of medications by maximizing the clinical effect, using the most effective treatment, minimizing the risk of treatment-induced adverse events, and minimizing the expenditures for pharmacological treatments.
  • Communicates findings and recommendations to other health care professionals who are responsible for the patient's care. Ensures that issues are resolved before dispensing.
  • Provide education/counseling to patients about drug therapies, and provides concise, timely responses.
  • Ensure the 7 patient "rights" are respected: right patient, right dose, right route, right time, right drug with the right information and documentation.
  • Report medication errors and participate in other aspects of improving safety in the organization.
  • Focus on compliance with regulatory requirements and organizational policies and procedures.
  • Undertakes projects that will improve the quality of patient care, in protocol development, promote cost effective use of medications, and pharmacy process improvements.
  • Learn and appropriately manage pharmacy automation, including inventory tracking and automated dispensing cabinets.
  • Participates in relevant committees.
    Assists in orientation and training of new hires, and precepts pharmacy students and residents.
  • Assumes personal responsibility for professional development, particularly in the area of ongoing clinical education, and develops clinical pharmacy programs according to policies and regulations.
