Role: Case Manager Navigator
Reports to: Program Manager
Location: Santos Plaza 3837 S. Western Ave., Los Angeles, CA 90062
Schedule: Monday - Friday, 8:00am - 4:30pm
Status: Full Time, Non-exempt (Hourly), Non-management

Summary

With the support of and in partnership with the E6 team, the inside Safe Case Manager-Navigator performs a variety of tasks to assist the most vulnerable and chronically homeless individuals living within the boundaries encampments identified and prioritized for hotel placements by the new Mayor's Inside Safe initiative. These teams are part of a new initiative by the Mayor's office to expeditiously move individuals in priority encampments into hotels and provide ongoing case management and housing navigation in order to quickly match them and move them into appropriate permanent housing resources. The Inside Safe case Manager will conduct outreach and also be responsible for the well being of client once they transition into their respective hotel placements. The selected candidate will be responsible for identifying, engaging and providing support to homeless clients, including providing documentation support, case management, housing navigation, and connection to medical/mental health services. Working as part of a team that will focus on specific encampments identified as high priority by the City and County, the selected candidate will work collaboratively to engage individuals that are living at these encampment sites, as well as provide linkage to housing by using a housing first model.

Essential Duties and Responsibilities

1. Work as part of a team to engage and provide services to homeless individuals.
2. Develop effective, trusting relationships with clients that facilitate provision of services.
3. Interface with partners, including community health providers, the business community, and social service/housing providers to reduce the number of homeless people living in specific encampments
4. Work in collaboration with other team members to remove barriers to permanent housing; e.g., acquisition of identification, legal aid, benefits assistance, life skills acquisition
5. Provide initial contact, needs assessment and triage planning
6. Assist all client obtain documents needed to complete housing applications
7. Ensure that all clients have been link to all benefits for which they are eligible
8. Refer targeted clientele to appropriate services (e.g., health, mental health, interim housing, etc.)
9. Identify housing stock to move individuals into housing as quickly as possible
10. Maintain good relationship with landlords and developers
11. Provide case management and aftercare/follow-up services to clients placed in permanent housing
12. Maintain and record accurate electronic records, client files, and statistical information.
13. Attend all case conference, staff meetings, trainings and retreats.
14. Engage individuals using a harm reduction and housing first principles
15. Work as part of a larger team composed five separate interdisciplinary teams.
16. Work collaboratively as a team to identify appropriate services taking into account each individual's physical and cognitive needs.

Qualifications

1. BA degree or 2 years equivalent experience working within homeless services.
2. Demonstrated knowledge of and experience with the issues faced by the population served (homelessness, mental illness, substance abuse, medical issues, and domestic violence).
3. Working knowledge of social services agencies in Metro LA (SPA 4) and LA County agencies serving the homeless.
4. Excellent human relations/communication skills; ability to act in professional manner regardless of circumstances.
5. Skilled in non-violent crisis intervention.
6. Ability to self-motivate and organize time effectively and efficiently.
7. Computer literate; able to use computerized database system for information management.
8. Passion for working with the population served.
9. Ability to work as part of an interdisciplinary team
