Family Health Centers of San Diego is looking for a Social Services Coordinator to provide coordinated social services in supporting patients entering FHCSD Primary Care System by addressing social determinants of health, providing referrals for support services, and guiding patients through FHCSD health systems.

Responsibilities:

  • Provide critical navigation services to patients and their family members, with exceptional customer service.
  • Assist and help in identifying needs of the patient for a successful primary care visit through facilitating complete discharge planning.
  • Perform patient assessments under protocol through use of evidence-based practices and tools.
  • Serve as a critical member of the multidisciplinary Care Team.
  • Accurately collect Social Determinants of Health, Sexual Orientation and Gender Identity, in addition to otheressential screenings to uncover patient needs and vulnerabilities.
  • Receive referrals and "warm handoffs" from the extended Clinic care team ranging from the waiting roomand rounding to promote patient linkage to care.
  • Develop and create primary care goals with the patient, and/or patient/caregiver as appropriate.
  • Develop annual SMART Goals with patients to include problems and needs, intended interventions and expected results.
  • Provide basic individual support based on patient's needs.
  • Qualify patients for existing FHCSD programs and facilitate linkage to include applicable program required enrollment that include a caseload of approximately 15 - 20 patients.
  • Effectively manage patient experience time to include exercising exceptional time management and communication skills.
  • Provide documentation of all encounters, assessments, and provided support.
  • Maintain good working relationships with community partners and utilize the community resources to connect new and existing patients to FHCSD.
  • Educate patients and their families on resources and problem-solving strategies on topics such as abuse, neglect, patient rights and responsibilities, conflict resolution, and community health and social services available.
  • Present a professional demeanor, be approachable, respectful and work well with others.
  • Always exercise high customer service (AIDET) and work closely with the clinic care team.
  • Converse and completely adhere to HIPAA regulations.
  • Assist with discharging patients when social services are needed before patient leaves the clinic.
  • Perform other duties as assigned.

Requirements:

  • Bachelor's Degree in Social Work required.
  • Master's Degree in Social Work strongly preferred.
  • Or equivalent combination of education and experience that provides the skills, knowledge considered and ability to perform the essential job duties, and which meets any required state or federal certification requirements.
  • Ability and means to travel as needed in a timely manner within San Diego County, to locations that may have limited access to public transportation; proof of liability and property damage insurance on vehicle used is required. DRIVER REQUIREMENTS: Licensed for a minimum of 3 years; No more than 2 violations and/or accidents within 3 years; No vehicle related suspensions/reinstatements; No DUI, reckless or felony Driving within 7 years.
  • 3 years minimum experience in healthcare or social services setting.
  • Intermediate computer literacy (i.e., use systems to manage and schedule appointments, access electronic medical records information, Microsoft suite, basic data analytics, high level key-boarding proficiency).
  • Bilingual in English/Spanish or English/Arabic preferred (language skills/competency will be tested).
  • Advanced written and verbal communication skills.

If you are hired at Family Health Centers of San Diego, your final base salary compensation will be determined based on factors such as geographic location, jurisdictional requirements, skills, education, and/or experience. In addition to these factors - we believe in the importance of pay equity and consider internal equity of our current team members as a part of any final offer. Please keep in mind that the range mentioned above is what we reasonably expect to pay for the role. Hiring at the maximum of the range would not be typical in order to allow for future and continued salary growth.
